What is ADHD?
Attention Deficit Hyperactivity Disorder (ADHD) is a neurodevelopmental condition that affects both children and adults. It can impact focus, self-control, and the ability to manage daily tasks effectively. People with ADHD may experience symptoms such as difficulty concentrating, impulsivity, and hyperactivity. Understanding ADHD is the first step to managing it, and an accurate diagnosis is crucial to getting the right support and treatment.
How to Refer Yourself for an ADHD Assessment

Sign Up on the ADHD 360 Website
To start your ADHD diagnosis journey, visit the ADHD 360 website at www.adhd-360.com. You'll need to sign up using your email address and NHS number.

Complete the RTC Referral Form
Once you've signed up, download and complete the RTC referral form available on the ADHD 360 portal. This is an essential step before we can proceed with your referral.

Email the Completed Referral Form
After completing the RTC form, email it to our Admin Manager, Mrs O Ciobanu. This will initiate your referral process.

Referral Confirmation
Within 21 days, we will confirm whether your referral has been processed and sent to ADHD 360. You will be notified when your referral has been accepted and the next steps are in motion.

Visit the ADHD 360 website at www.adhd-360.com. You'll need to sign up using your email address and NHS number.
What Happens After Your Referral is Accepted by ADHD 360?
Once ADHD 360 accepts your referral sent by Mrs O Ciobabu, you will now be under the care of ADHD 360.
For any queries or concerns regarding the referral, you should contact ADHD 360 directly.
There is no need to contact the practice to chase up the referral; all follow-up inquiries should be directed to ADHD 360.
Once acepted ADHD 360 will contact you to arrange an assessment. You will either choose an available date from the clinic’s portal (Chrysalis) or work with their team to schedule a time that suits you.
Pre-Assessment Information
Unlike standard NHS processes, ADHD 360 gathers detailed information ahead of time, making the assessment process more efficient and thorough. This allows for a high-quality consultation that is tailored to you. If you are referring a child for assessment, you may want to complete the Children’s Screening Tool and take it to the school for completion. The more information available, the more accurate the assessment will be.
The ADHD 360 Assessment Process
Your ADHD assessment will be a semi-structured conversation with a Clinician who specialises in ADHD. The Clinician will gather information, ask questions, and ensure that your experience is positive and supportive. If the patient is a child, we will ensure they are asked questions in their own right and encourage open communication with both the child and their family during the consultation.
Getting Your Diagnosis
At the end of the assessment (or sometimes earlier), your Clinician will provide a diagnosis of ADHD or explain why it was not diagnosed. If diagnosed, this marks the beginning of your journey with ADHD, and ADHD 360 will be there to support you through your treatment. If you are not diagnosed, they will guide you on what to do next.
